Altitude Sickness First Aid Kit for Ladakh

Flying into Leh lands you at 3,524m — higher than most peaks in the Alps — within 80 minutes of leaving Delhi. Your body has had no time to adjust. Most visitors feel something in the first 24 hours: a mild headache, a touch of dizziness, an unusual fatigue. This is normal. It is your cardiovascular system working harder to move oxygen around at reduced atmospheric pressure.

A well-prepared altitude first aid kit does not eliminate AMS. Nothing does — acclimatisation takes time, and the process is the same whether or not you carry medication. What the kit does is give you effective tools to manage common symptoms, monitor your status, and know when the situation requires more than rest.

The core kit: what to carry

Paracetamol (acetaminophen)

The altitude headache is almost universal among first-time Leh arrivals. Paracetamol — standard dosage, same as for any headache — reliably reduces it. It does not treat the underlying cause (low oxygen), but it manages the symptom while your body acclimatises. Take it early when the headache starts rather than waiting for it to intensify.

Ibuprofen is an alternative. Some altitude medicine research suggests ibuprofen may have modest prophylactic effects against AMS beyond simple pain relief. It is more irritating to the stomach than paracetamol and should be taken with food. Either works for the headache; both are widely available in Leh if you run out, but carry your own supply for the first few days.

Oral rehydration salts (ORS)

Altitude increases fluid losses through the respiratory system — breathing harder and faster at altitude, even at rest, accelerates dehydration. The standard advice to "drink plenty of water" at altitude is correct. ORS sachets — a pre-mixed balance of salts and glucose — support hydration more effectively than plain water when dehydration is already present or when appetite is suppressed.

Carry a supply of ORS sachets. They pack flat, weigh almost nothing, and are significantly more effective than water alone when you feel rough and are not eating well. The first day in Leh is the most likely time to need them.

Pulse oximeter

A pulse oximeter clips to your fingertip and measures SpO2 — blood oxygen saturation — in seconds. This is the single most informative piece of altitude monitoring equipment you can carry.

At sea level, a healthy adult has an SpO2 of 96–99%. In Leh, values of 88–94% are normal and not concerning in themselves. The number to watch is trajectory — if your SpO2 is declining over hours rather than stabilising, or if it drops significantly with minimal exertion and doesn't recover, this is more informative than the absolute reading.

Diamox (acetazolamide) — prescription required

Acetazolamide — brand name Diamox — is the medication most commonly associated with AMS prevention and treatment. It works by stimulating faster and deeper breathing, which raises blood oxygen levels and accelerates acclimatisation. It does not prevent AMS absolutely; it reduces severity and shortens duration in most users.

Diamox requires a prescription. Consult a doctor before departure — not on arrival in Leh. Your doctor can assess whether it is appropriate for you, advise on dosing, and flag contraindications (it is a sulphonamide derivative; those with sulfa allergies should not take it). Common side effects include increased urination, tingling in fingers and toes, and altered taste of carbonated drinks. These are unpleasant but not dangerous.

Additional items worth carrying

Antihistamine

Dry air and altitude can trigger allergic symptoms in some visitors — nasal congestion and irritation that compounds the altitude headache. A non-drowsy antihistamine manages this without the sedation that would compound altitude fatigue. It also doubles as a mild sleep aid if drowsiness is preferred on a first acclimatisation night.

Antiseptic and wound care

Altitude slows wound healing. The reduced oxygen availability and UV exposure both contribute. Minor cuts and abrasions heal more slowly at 3,500m than at sea level — keep wounds clean and covered more diligently than you would at home. Antiseptic cream, plasters in multiple sizes, and medical tape cover the basics.

Anti-nausea medication

Nausea is a common AMS symptom. Ondansetron (where you have a prescription and your doctor recommends it) or over-the-counter options like ginger tablets address the symptom without sedation. Nausea that prevents eating or drinking is particularly problematic at altitude — managing it matters because hydration and food intake directly support acclimatisation.

Throat lozenges and lip balm

The air in Ladakh is exceptionally dry. Breathing through slightly opened lips to get more air — which many people do automatically at altitude — desiccates the throat and lips faster than you expect. Medicated throat lozenges for the sore throat that arrives on day two are more useful than they might sound. Lip balm with SPF for the UV protection is essential regardless.

What to do when the kit is not enough

The most important altitude first aid principle is the one that requires no equipment: descend. If symptoms are progressing — headache not responding to paracetamol, vomiting that prevents hydration, breathlessness at rest, confusion, loss of balance, or SpO2 declining despite rest — descent is the treatment. Going down 300m to 500m in altitude produces measurable improvement in hours.

Paracetamol, ibuprofen, ORS sachets, antihistamines, and basic wound care supplies are all available in pharmacies in Leh city. Pulse oximeters can sometimes be found in medical supply shops. Diamox requires a prescription — bring it from home if you plan to use it, rather than hoping to source it on arrival.

Beyond Leh city, pharmaceutical options thin rapidly. Nubra Valley has some basic facilities; the Changthang plateau does not. If you are heading to Pangong, Tso Moriri, or Hanle, take everything you might need from Leh — the altitude is higher, the exposure is greater, and medical infrastructure is minimal.
